Overview
The Rural Health Transformation (RHT) Program is a nationwide, five-year, $50 billion federal initiative designed to strengthen health care access, delivery, and outcomes in rural communities.
Virginia received $189,544,888 for budget year one to advance a comprehensive, community-driven strategy to improve rural health across the Commonwealth. States will receive a new federal award amount over 5 budget periods during the program.

CareIQ
CareIQ will deploy innovative technology and data-driven solutions to:
- Modernize systems and interoperability
- Improve patient outcomes
- Reduce provider burden
Homegrown Health Heroes
This initiative strengthens the rural workforce by:
- Expanding training and education pathways
- Recruiting and retaining clinicians and allied health professionals
- Building pipelines that develop local talent to serve local communities
Connected Care, Closer to Home
This strategy brings flexible, sustainable care models directly into communities by:
- Expanding telehealth and mobile care
- Supporting hybrid care delivery models
- Reducing avoidable emergency department visits
Live Well Together
Live Well Together promotes healthier communities by:
- Expanding prevention efforts
- Reducing chronic disease risk factors
- Increasing access to healthy food and wellness resources
- Empowering Virginians to lead healthier lives through education and self-management
Key Implementation Partners
Virginia’s RHT Program is supported by Key Implementation Partners, who will lead and manage multiple sub-initiatives in collaboration with state agencies, providers, and community organizations.
